Coming back to Hong Kong from Canada, Po-erh has to realize that not evverything is alright at her parents home: Her mom reacts deeply disturbed, even fearful, towards her, she suffers from terrible headaches, and on several occasions she sees the spirit of a young girl. It soon turns out that the spirit is Bei-erh, the Siamese twin Po-erh was seperated from when she was little & who died after surgery, & who has now turned into a vengeful entity - But the only persons who know about the operation are Po-erh's parents & doctor Cheung, & they are reluctant to tell her. The only person really on her side is handsome doctor Poon, who soon finds out her medical history, which he informs her about, but refuses to believe in Bei-erh's spirit haviong come back to haunt Po-erh. Meanwhile, Po-erh's mother gets in a fight with her father & doctor Cheung during which she is hurt, & Bei-erh - only feeling loved by her mother anyways - decides to have them being involved in a car crash, in which both the father & Cheung die. Next Bei-erh possesses Michael, who is in love with Po-erh, by taking on Po-erh's appearance & having sex with him, & soon he goes after Po-erh, to be killed in the nick of time by Wendy, the girl who he was caught having sex with by Po-erh. Wendy & Po-erh decide to go to doctor Poon for help, but he already had a visit by Bei-erh, who - in the form of Po-erh - had sex with him too & now possesses him. He also goes after Po-erh, but only manages to kill Wendy, & when being sprayed by her blood, returning to normal. Now he & Po-erh decide to leave Hong Kong with her mother, who even agrees to that plan but secretly has other plans : She wants to kill herself & Bei-erh in a gas explosion, & it seems to work ...

At the airport, Poon anxiously awaits Po-erh, who comes literally in the last minute ... but twice. Out of desparation, Poon kills the Po-erh he thinks is indeed Bei-erh in public, only to find out it was the real Po-erh after all ...

This very downbeat ending is - though not all that logical - the best scene of this horror film that comes across very slick, but totally devoid of suspense or mystery - when it is finally revealed that Po-erh is in fact one half of Siamese twins, in fact everything was already pointing to it for about half an hour: Not only is the film called Siamese Twins, it also has a lengthy prologue explaining facts about siamese twins as well as their mortality & chances for their separation, having the audience guess right from the beginning who of the protagonists might be one half of a Siamese twin-couple ... & Po-erh really is the only possible candidate.
